Medication Therapy Management

Medication Therapy Management, often shortened to MTM, is a term used for medication-focused review and support. A medication review can help a patient organize questions about prescription medicines, nonprescription products, and how different parts of a medication list fit together.

Preparing for a medication review

A useful starting point is a complete and current medication list. Depending on the situation, that list may include prescription drugs, over-the-counter medicines, vitamins, and supplements. It can also help to have current medication labels available.

Questions a patient may want to prepare include:

Medication questions and the prescribing clinician

A pharmacist can be an important source of medication information, but a pharmacy review does not replace the clinician responsible for prescribing and managing an individual treatment plan. Questions about changing a prescription, treatment goals, or a medical diagnosis should be directed to the appropriate prescriber.

Confirm current availability

MTM and medication-review services can vary by pharmacy and by patient circumstances. Contact King Pharmacy to confirm whether Medication Therapy Management or another medication-review service is currently available and what information should be brought for the discussion.
